About the Role

Our client is seeking a dedicated and skilled Community Health Nurse to serve the community of Mossel Bay . In this vital role, you will provide essential primary healthcare services, health education, and preventative care directly within the community, focusing on improving health outcomes for individuals and families. You will work autonomously and as part of a team to address public health needs, conduct screenings, and offer support for chronic disease management. Key Responsibilities Provide comprehensive primary healthcare services, including consultations, assessments, and treatment for common ailments. Conduct health screenings, immunizations, and wellness programs for various community groups. Educate individuals and families on health promotion, disease prevention, and management of chronic conditions. Perform home visits to assess patient needs, provide care, and monitor progress. Collaborate with local clinics, hospitals, and community organizations to coordinate patient care. Maintain accurate patient records and manage essential medical supplies. Requirements Registered Nurse qualification with a valid South African Nursing Council (SANC) license. Experience in community health nursing or primary healthcare settings. Strong clinical assessment skills and knowledge of public health principles. Excellent communication and patient education skills. Ability to work independently and manage time effectively in a community-based setting. Proficiency in relevant EMR systems and general computer literacy.
